|Drug Name and Strength||Cephalexin (as monohydrate) 600 mg|
|Indication||Infections due to susceptible organisms and conditions, including pyoderma, abscess, urinary tract infections, trauma, osteomyelitis, surgical prophylaxis and respiratory infections in cats and dogs.|
|Actions||Cephalexin is the antibiotic of choice for conditions where longer-term therapy is required, such as tenacious pyoderma and abscesses. This is reinforced by the bactericidal activity of cephalexin against Staphylococcus intermedius, including β-lactamase producing strains. Its broad spectrum provides activity against secondary infecting bacteria such as Proteus or Escherichia coli, providing more therapeutic success.Because it is secreted unmetabolised in the urine, Rilexine provides high and sustained antibiotic levels for the treatment of urinary tract infections (UTI). These levels are minimally affected by variations in urinary pH often experienced in UTI. Urinary levels achieved by cephalexin of greater than 225 microgram/mL provide effective treatment against the major bacteria encountered in UTI.Cephalexin provides a superior choice for trauma cases because of its broad spectrum of activity and excellent diffusion into bone and joints. Rilexine is indicated in the treatment and prevention of osteomyelitis.Cephalexin is also indicated for surgical prophylaxis because of its documented activity against the major Gram-positive and Gram-negative organisms. Cephalexin does not interfere with anaesthetic procedures.Cephalexin is bactericidal and exerts its activity by destroying the integrity of cell walls. The β-lactamases are enzymes, secreted by a number of bacteria, which degrade penicillin by hydrolysis. Typically these bacteria include strains of Staphylococcus and enterobacteria.The majority of these β-lactamases are of the penicillinase type which are unable to degrade cephalexin. The cephalexin molecule incorporates a dihydrothiazine chain near the β-lactam cycle, making it much more stable than the penicillin molecule, which incorporates a thiazolidine cycle. This stability is accompanied by greater activity (cephalexin is 500 times more effective than benzylpenicillin against Staphylococci) and a much broader spectrum of activity.Toxicity of cephalexin is low, regardless of administration route (LD50 is more than 1,000 mg/kg). Unlike the penicillins, allergic reactions to cephalexin are extremely uncommon.Microbiology Cephalexin is a cephalosporin, a family of semisynthetic antibiotic agents chemically related to the penicillins. Cephalexin has considerable advantages over the penicillins because of its natural resistance to β-lactamases. It also has a broad spectrum of action.Pharmacology Cephalexin, when used orally, is almost completely absorbed (> 85%) in the anterior part of the gastrointestinal tract providing the additional benefit of not disturbing normal intestinal flora. The poor binding of cephalexin with plasma proteins (15%) accounts for the excellent tissue diffusion and tissue life achieved. The tissue half-life of cephalexin is four times higher than its serum half-life in the dog.Pharmacokinetics Cephalexin is not metabolised. It is eliminated in the urine under its active form by glomerular filtration and active tubular secretion. This urinary concentration allows very high levels of cephalexin in the urine (50 to 100 times higher than plasma levels). The activity of cephalexin is virtually unaffected by variation in urinary pH.|
|Dosage and Administration||Rilexine 75 mg. Cats, dogs: 15 mg/kg twice daily (1 tablet/5 kg bodyweight morning and night) or 30 mg/kg bodyweight once a day (2 tablets/5 kg bodyweight at 24 hour intervals). Rilexine 300 mg. Dogs: 15 mg/kg twice daily (1 tablet/20 kg bodyweight morning and night) or 30 mg/kg bodyweight once a day (2 tablets/20 kg bodyweight at 24 hour intervals). Rilexine 600 mg. Dogs: 15 mg/kg twice daily (1 tablet/40 kg bodyweight morning and night) or 30 mg/kg bodyweight once a day (2 tablets/40 kg bodyweight at 24 hour intervals). Long-term treatment may be necessary for skin and urinary tract infections.Duration of treatment is at least three days but may be extended at the discretion of the veterinarian.Cephalexin is active against certain strains of penicillinase producing Staph. aureus and also effective for use in urinary infections caused by susceptible bacterial species.|
